Before treatment:

The patient suffered from language deficit when he was 1 year and 6 months old. He had less communication with others. He bites his fingers. He also had stereotyped act and a flirtatious mood. The patient went to school when he was 3 and a half years. He had difficulty with sitting quietly. He always runs and fights in classroom. The patient left normal school after one year of admission. He went to many hospitals and received head MRI and EEG. He was diagnosed with Attention deficit hyperactivity disorders. He received Ritnosin for 15 days, Gamalate B6 45 days and haloperidol for 30 days. He also received Phenergan for therapy. But his disease was not alleviated. Some of the therapy even made the disease worse and he had severe side effect. The patient started with language rehabilitation and behavioral therapy 14 years ago. About 13 years ago, the patient received ADHD assessed and showed the patient suffered from slight hyperactivity. PIMS autism assessed the patient, concluded that the patient suffered from Autism. So the patient continued with language rehabilitation and behavioral therapy. The patient went to a special hospital, his speech and behavior were improved.  

From the onset of disease, the patient had slight hyperactivity, polydipsia, polyphagia and diuresis. He liked sweets and defecates 3 times every day. The sleep was not regular. There was no genetic disorders.

Admission PE:

The patient couldn't cooperate with the examination of vital signs. The development and nutrition were normal. The body type was normal. The respiratory sounds in both lungs were clear, with no dry or moist rales. The heart sounds were strong; the rhythm of his heartbeat was normal, with no obvious murmur in the valves. The abdomen was flat and soft, with no pressing pain or rebound tenderness. The liver and spleen were not palpable under the ribs. Doctor also monitored his heart rate frequently and it was between 120/min to 140/min.

Nervous System Examination:

Ahsan Amer was alert and had a nervous spirit. He suffered from mild hyperactivity. His intelligent was damaged. His language expression was limited and only could say 200 to 300 words. He answers question with simple words (mainly was 1 to 3 words). He had less active communication with others. He mainly answers his father's question and follow his father's simple orders. He would answer question after repeatedly prompted. He could understand routine matters, such as routine items, persons and peoples around him. He needed assistance to clean himself. He could dress himself and feed himself. If he found there were others present, he would like to depend on them. He couldn't watch TV, couldn't make eye contact with others. If he was in a good mood, he would look at person quickly with the corner of his eye. He liked to tear newspaper. The muscle strength and muscle tone were normal with initial observation. He refused bodily touch except slight touch. He always bend elbows and raises arms in front of face. He couldn't cooperate with the examination of cranial nerve, tendon reflex, pathological reflexes, coordinate movement, sensory system or meningeal irritation sign.

Treatment:

After the admission, the patient received the relevant examinations. The patient was diagnosed with Autism. The patient received treatment for nerve regeneration and to activate stem cells in vivo. The patient also received treatment to improve the blood circulation and also to nourish his neurons. This was accompanied with physical rehabilitation.

Post-treatment:

The patient's polydipsia and diuresis are alleviated. Heart rate reduced to 95/min to 110/min. He has better ability to communicate with others.He can answer questions when asked by a stranger. He has better ability to answer family members question and follow simple orders. He needs less motivation before answering questions. He has better cooperation with physical examination. The action of bending elbows and raising arms in front of face has reduced obviously. He has better self-care ability of daily living.

He is putting up  rapidly weight along with swallowing some time on his face. And most of time
he appears to be fatigued and lethargic, and unwilling to go out for walk and any game. Most of time he like to lay down in a bed  and go to half sleep. In morning he takes lot of water and his hands are little frozen cold and in night he takes heavy breathing with uncomfortable noise. He has also developed reddish spot on his head and also to his both legs. Which itches and he has scrapped his legs with his nails of hand.His head have now recently developed white colour removal dry skin may be dandruff but in huge quantity.

Please advise what to do now, shall we continue the same dose or you send new one?

      His eye contact has improved and now he started watching the TV also for 5 to  10  minutes with very focused eye contact.
He has also reduced the urination to 10% and water intake may be because of cold weather.

Its almost four months gone without change in above said patient treatment please advice if any change required.

Following are few latest informations for your perusal.

1) Ahsan Blood Pressure trend in last four weeks is as follow:-

   Lowest     118/72     after 30 minutes of dosage.

   Highest    135/90     after 10 hours of dosage.

   Average    130/80     its based on after and before dosages.

2) His Pulse rates are as follow.

     Lowest    90           after 30 minutes of dosage.

     Highest    112         after 10 hours of dosage.

     Average    102        mix of after and before.

3) ORYZANOL.   Dear doctor Oryzanol is short with us since two weeks
    Ahsan skin rashes have on its own started reducing and his skin colour
    have also improved to better. I request you to correlated this with withdrawal
    of ORYZANOL? and reply and advice me accordingly.

4) Ahsan HEADACHE is also reduced.

5) His sleep is around 10 hours.

6) His vocabulary has improved drastically and behaviour to respond the command
    has also improved. Meaning by now he if been told to do some work like pick
    up the jug of water from the table of dinning room and place it in kitchen he
    does it. Before this he use to do but mostly on mood.

7) His interest has developed to watch Television continuously for 10 to 20 minutes with focused eye contacts.Simply his eye contact has improved.

8 ) Please see as attached 23 required latest blood test results for further
     prognosis.
