Carlos Alonso-Injury of the left sciatic nerve-(Argentina)

Diagnosis: Injury of the left sciatic nerve

Before treatment:
The patient received femoral head replacement of the left lower limb. Then the patient lost sensation and motor ability of the left lower limb. After rehabilitation training, part of the sensation in the left lower limb's near-end was gradually restored. There was no response below the left knee joint. The left foot dropped and needed correcting. The local hospital doctor thought the artificial thigh bone maybe hurt the nerves. For the infection of the artificial thigh bone, the patient received femoral head replacement again 2 years ago. The sensory of the left lower limb had no change post-operation. 9 months later, the patient started with rehabilitation training. The sensory and muscle strength of the left lower limb had recovered some. The patient received a local implantation of autologous bone marrow stem cells, but this had no effect. Before the treatment, the patient could walk, but the walking gait was abnormal. The left foot dropped. The sensory below the left knee joint was obviously slowed down. There was obvious atrophy in the left gastrocnemius muscle.

Admission PE:
Bp: 109/63 mmHg; Hr: 57/min. The nutrition was normal. The examination of the heart, lung and abdomen showed no abnormalities. There was two red papules in the calf of the left leg. The diameter of the red papules in the left calf's far-end was 4cm and in the left calf's near-end was 2cm. There was no desquamation or seepage.

Nervous System Examination:
Carlos Alonso was alert and his speech was clear. The orientation, calculation abilities and memory were normal. Both pupils were equal in size and the diameter of both pupils was 3.0mms. Both pupils were sensitive to light stimuli, and both eyeballs could move freely. The forehead wrinkle pattern was symmetrical. The tongue was in the center of the oral cavity and the teeth were shown without deflection. The neck could be turned in both directions flexibly. The muscle strength of both upper limbs was level 5. The muscle tone of both upper limbs was normal. The left lower limb presented with a leaping gait. The left foot drop and the muscle strength of the left foot was level 0. There was spontaneous pain in the left foot. The thighs and legs of the left lower limb was level 5. There was obvious atrophy of the gastrocnemius. The muscle strength of the right lower limb was level 5. The muscle tone of both lower limbs was normal. The deep sensation and shallow sensation of left L5-S1 was slowed down obviously. The fine sensation was almost normal. Other deep sensation, shallow sensation and fine sensation were normal. The tendon reflex of both upper limbs was elicited normally. The patellar tendon reflex of the left lower limb was slowed down and the tendon reflex had disappeared. The tendon reflex of the right lower limb was normal. The abdominal reflexes were not elicited. Bilateral cremasteric reflex was sensitive. The sucking reflex was negative. Bilateral palm jaw reflex was negative. The pathological sign of the four limbs was negative.  There were no signs of meningeal irritation. The coordinate movement examination was normal. The laboratory test showed the gamma globulin was reduced.

Treatment:
We initially gave Carlos Alonso a complete examination, and he was diagnosed with an injury of the left sciatic nerve. He was then given treatment in order to expand the blood vessels, to promote nerve growth and nourish the neurons. He also received treatment for nerve repair and nerve regeneration. This was combined with physical rehabilitation treatment.

Post-treatment:
The patient suffered from low levels of globulin. After replenishing the gamma globulin, the levels of globulin were restored to normal. The motor ability of the left lower limb has increased. The leaping gait was not so severe. The left side gastrocnemius volume has increased. There appears to be a weak ankle reflex of the left lower limb. The rash in the left lower limb has healed. The spontaneous pain in the left lower limb has disappeared.

Lisa! Carlos is fine, his doctor says he's got more muscles and that he is stronger than before.
He needs to know which studios has to be done to him, and the medication he needs to take... he's got everything in a folder but he lost it in a taxi!!!
