Before treatment:
One year ago, the patient felt weak in right upper limb, her neck was painful, then she felt weak in left lower limb and she also had balance problems, she fell a lot, so she went to local hospital and did EMG, she was diagnosed with ALS, she took Riluzole and other medicines without effect. 6 months ago, she was unable to speak clear, 4 months ago, it was hard for her to swallow, she choked when drinking, she also had tremor of tongue, her four limbs were weak, she was unable to take care of herself. At present, her four limbs are weak, to make a further treatment, she comes to our hospital.
Her spirit is not good, she eats liquid diet, her sleep is good, she is unable to control urination well, she has constipation, sometimes she has headache, she loses some weight recently.

Admission PE:
Bp: 180/70mmHg, Hr: 95/min, breathing rate: 20/mim, body temperature: 36 degrees. Nutrition status is good, normal physical development. She was sended to the patient room by wheelchair, there is no injury or bleeding spots of her skin and mucosa, no blausucht, no throat congestion. Chest develop is normal, the respiratory sounds in both lungs were low, respiratory phase extended, no dry or moist rales. The heart beat is strong with regular cardiac rhythm, with no obvious murmur in the valves. The abdomen was soft and plate, with no masses or tenderness. The liver and spleen were normal, shifting dullness was negative, spine column is normal, there are pitting edema in below both ankle joints, skin was red, the temperature was normal.

Nervous System Examination:
Patient was alert and mental status was weak, dysarthria, the memory, orientation and calculation ability were normal. Both pupils were equal in size and round, diameter as 4.0 mm, react well to light, eyeballs can move freely. Bilateral forehead wrinkle and nasolabial groove is symmetrical, she can make tongue out to lips side, tongue muscle atrophy, and the movement ability decreased. She could not show teeth as normal. Bilateral soft plate could not lift as normal, chewing ability was weak, swallowing difficulty, she only took semiliquid food, and she had chock occasionally. There were middle muscle atrophy in the bilateral shoulders, upper limbs proximal side, distal side, both thenar muscles, neck was soft, shrug ability was slightly decrease, muscle power of upper limbs was 1 degree, left hand fingers can move slightly, right hand fingers could not stretch well, muscle power of lower limbs were 1 degree, muscle tone of upper limbs were normal, of lower limbs was higher. Tendon reflex of 4 limbs were normal. Bilateral Palm-jaw reflex were positive, Hoffmann sign of both sides were positive, bilateral Babinski sign were negative, patient could perform the finger to nose test and fast alternate movement slowly, she could not perform the finger opposite movement and lower limbs coordinate movement because of the weakness, the Meningeal irritation sign is negative.

Treatment:
After the admission, she received related examinations and diagnosed with ALS. She received 3 neural stem cell injections and 3 mesenchymal stem cell injections to repair her damaged nerves, replace dead nerves with new injected stem cells, nourish nerves, regulate her immune system and improve blood circulation, with rehabilitation training.

Post-treatment:
After 14 days’ treatment, her speech was much louder and clear than before, she ate faster and had less choke, pharyngeal reflex was much better, muscle power of upper limbs was increased 20%, her limbs were flexible, she grasped powerful, muscle power of lower limbs was increased 1 level, she was able to move legs on bed, her knee joints were able to raise from bed, the feet movement were much flexible. the breathing function was much stable than before, saturation oxygen was increased to 95-97%.
