Before treatment:
Mordechay Misheyeb had fasciculation in four limbs 3 years, he wasn’t diagnosed, 2 years ago, he couldn’t speak clear. In June 2016, his right hand was weakness, it was hard for him to grasp, his left hand was weakness too, he went to local hospital and did EMG, he was diagnosed with ALS. His four limbs were weakness, there were muscular atrophy. He was unable to speak clear, he had short of breath, it was hard for him to turn over, go up and down stairs. He did treatment 6 months ago in our hospital, and after the hospital his lower limbs were better, but for now, his condition becomes worse, he is unable to lie down, he has respiration problems. He wants a better treatment, so he comes to our hospital.
His spirit and sleep are normal. His appetite is bad, his urination function is normal. He lost 2 kgs. He had diabetes type 2 for many years.

Admission PE:
Bp: 125/81mmHg; Hr: 78/min. Br: 18/min. Body temperature: 36.5 degrees. Height：156cm，weight：70 kg. His development was normal，he had good nutrition. There were no yellow stains or petechia on the skin or mucous. Barrel chest，the thorax was in symmetry. The movement range of his chest was reduced. The breath sound of his bilateral lower lung was low with no dry or moist rales. The rhythm of the heartbeat was normal. The heart sounds was strong. There was no obvious murmur in the valves. The abdomen was flat with no pressing pain or rebound tenderness. Shifting dullness was negative. Spine physiological bending was existed. The liver and spleen were normal. There was no edema in both lower limbs.

Nervous System Examination:
Mordechay Misheyeb was alert and his spirit was good. He had dysarthria, slur speech. His memory, calculation abilities and orientation were normal. Both pupils were equal in size and round, the diameter was 3 mms. Both eyes had sensitive responses to light stimuli. Both eyeballs could move freely. There was no nystagmus. The nasolabial fold and forehead wrinkle pattern were symmetrical. His tongue was centered in the oral cavity, he could make tongue out to lip 0.5 cm, tongue muscle atrophy with limited movement. His tongue could not touch the cheek powerfully, he could not bulge the cheek. Chewing ability was fine. The soft plate of both sides could lift, the uvula was toward to the right. The muscles were atrophied of his bilateral shoulder girdle，both upper limbs，bilateral thenar and  hypothenar and interosseous of hand. His neck was soft，the power of shrug shoulders and turn head were weak. His endurance was reduced. He could turn-back slowly. Both upper limbs abductor muscle power was 5-degree, extensor and flexor muscle power were 4-degree, bicipital muscle power was 3+degree, grip force of left hand was 4 degree, of right hand was 4-degree. Muscle power of left lower limb was 4- degree, of right lower limb was 3+ degree. Muscle tone of 4 limbs were normal. The tendon reflexes of four limbs were normal. Bilateral Hoffmann and Babinski sign was negative. Finger-to-nose test and finger opposite movement were normal. The rapid alternating movements were slow and clumsy. The meningeal irritation sign was negative.

Treatment:
After the admission, he received related examinations and diagnosed with Amyotrophic Lateral Sclerosis and diabetes (type2). He received 3 neural stem cell injections and 3 mesenchymal stem cell injections to repair his damaged nerves, replace dead nerves with new injected stem cells, nourish nerves, regulate his immune system and improve blood circulation, with rehabilitation training.     

Post-treatment:
After 15 days’ treatment, his short breath symptoms got better, Br:18/min, he was able to lie down, he spoke clear and fluent. The muscle power of upper limbs improved, the extensor muscle power was 4+ degree. His exercise tolerance was better, he walked better.

There were no yellow stains or petechia on the skin or mucous. He was barrel chested. The thorax was in symmetry but the movement range of his chest was reduced. The breathing sound of his bilateral lower lung was low with no dry or moist rales. The rhythm of the heartbeat was normal. The heart sounds were strong and there was no obvious murmur in the valves. The abdomen was flat with no pressing pain or rebound tenderness. The liver and spleen were normal. There was no edema in the legs. The skin temperature was normal. Sat was 90-93%.

Nervous System Examination:
Mordechay Misheyeb was alert and his spirit was good. He had dysarthrosis. He was not able to speak clearly. His memory, calculation abilities and orientation were normal. Both pupils were equal in size, round and the diameter was 3 mms. Both eyes showed sensitive responses to light stimuli. Both eyeballs could move freely. There was no nystagmus. The nasolabial fold and forehead wrinkle pattern were symmetrical. His tongue was centered in the oral cavity and the tongue muscle was not atrophied. The teeth were shown without deflection. The tongue muscle movement was limited，his tongue could not push strongly against the cheek but he was able to do cheek blowing well. His chewing ability was normal. Both soft palates could be raised, the uvula was toward to the right. The muscles were atrophied on his bilateral shoulder girdle, both arms, bilateral thenar and  hypothenar and the interosseous of his hand. His neck was soft but the power of shrugging the shoulders was weak. His endurance was reduced and he had difficulty in walking up and down stairs. His movement in turning over was slow. His extensor and flexor muscle strength of the right arm was level 4. His extensor and flexor muscle strength of left arm was level 5-. The grip strength of the hands was level 3+. The muscle power of his left leg was level 3，the muscle power of his right leg was level 2+. The muscle tones of the four limbs were normal. The tendon reflexes of the four limbs were normal. Bilateral Hoffmann and Babinski sign was negative. Finger-to-nose test was normal. The rapid alternating movements were slow. The little finger of his right hand could not finish the finger-to-finger test. The finger-to-finger test of the left hand was normal. He could not finish the heel-knee-tibia test. The meningeal irritation sign was negative.

Treatment:
After  admission, he was diagnosed with Amyotrophic lateral sclerosis. He received 3 neural stem cell injections and 3 mesenchymal stem cell injections to repair his damaged motor nerves, replace dead nerves with new injected stem cells, nourish nerves, improve blood circulation and regulate his immune system.  He undertook rehabilitation training.     

Post-treatment:
After 14 days of treatment, his respiration function has improved, his Sat is 94-96%. His speech is better. His little finger on the right hand can finish the finger to finger test，the rapid alternating movement is more flexible. The powers of his lower limbs are stronger and can now reach level 4.
