Before treatment:
Tadeusz Rutkowski felt weakness and pain in his back 2 years ago. He tired easily, his legs became weak, he walked slowly and his condition soon became worse. His arms became weak too and he also had muscular atrophy. In February 2016, he did MRI and EMG examinations and was diagnosed with Amyotrophic lateral sclerosis. He received three mesenchymal stem cell injections in Poland but there was no positive change. At present, his four limbs are weak, he needs helps with eating and dressing. He is able to sit up, stand up and walk 20-30 steps slowly. His exercise tolerance is low. He wants a better life so he came to our hospital.
His spirit, diet and sleep are good. His bladder and bowel functions are normal. He has lost around 10 kgs in 2 years.
Admission PE:
Bp: 124/82mmHg, pulse: 62/min, breathing rate: 19/min. Body temperature: 36.7℃. HT: 178cm; WT: 83Kg. Nutrition status and physical development were normal. There was no bleeding spots on his skin and mucosa, no cyanosis on his lips, no congestion in pharyngeal or swelling of his tonsils. The thorax was symmetrical. The respiratory sound of lungs was normal with no dry or moist rales. There was no upheaval in the precordium. The heart beat is powerful with regular cardiac rhythm and no obvious murmur in the valves. The abdomen was soft and flat, with no masses or tenderness. The spleen and liver were normal under palpating touch. The legs had no edema. Sat was 93-95%.
Nervous System Examination:
The patient was alert but with slurred speech. His memory, orientation and calculation abilities were normal. Both pupils’ diameter is 3mm. They reacted normally to light stimulus and the eyeballs moved normally. There was no nystagmus. Bilateral forehead wrinkles and nasolabial fold were symmetrical. When stuck out, his tongue was in the middle and he had symmetrical teeth. His tongue was able to touch his cheeks normally. There was no air leaking when he blew out his cheeks. The chewing ability was strong and he raised his soft palate normally. He could close his eyes normally. Bilateral shoulder girdle, arm muscles and thenar had atrophy. He could turn his neck and shrug shoulders normally. Left arm muscle: the upper arm adduction and abduction muscle strength was level 4, forearm flexor muscle strength level 3+, forearm extensor muscle strength level 3-, left hand grip strength level 4. Right arm muscle strength: the upper arm adduction and abduction muscle strength was level 3, forearm flexor muscle strength level 3, extensor level 2, right hand grip strength level 4-. Left leg muscle strength level 3, right leg muscle strength level 2+. The muscle tone of the four limbs was normal. The patellar tendon reflex cannot be elicited. Bilateral palm-jaw reflex was positive. Bilateral Hoffmann sign was negative. Bilateral Babinski was negative. The finger-to-nose test, rapid rotation test and finger-to-finger test were flexible. The knee-shin-heel test was normal. Meningeal irritation sign is negative.
Treatment:
After the admission he was diagnosed with Amyotrophic lateral sclerosis. He received 3 neural stem cell injections and 3 mesenchymal stem cell injections to repair his damaged motor nerves, replace dead nerves with new injected stem cells, nourish nerves, regulate his immune system and improve blood circulation. This was done along with rehabilitation training.
Post-treatment:
After 11 days of treatment, Tadeusz’s breathing function improved. The oxyhemoglobin saturation rose to 98%-99%. His spirit and energy are both improved. The muscle strength of legs has improved. The walking gait has improved. Left arm muscle strength: upper arm adduction and abduction are level 4+, forearm flexor and extensor is level 4 and level 4-, grip strength level 5. Right arm muscle strength: upper arm adduction and abduction are level 3+, forearm flexor level 3+, extensor level 3-, grip strength level 5-; left leg muscle strength level 4, right leg level 3-.
